1. Parent or guardian authorization
I certify that I am at least 18 years old and am the athlete’s parent or legal guardian, with authority to give these permissions. The information I provide is accurate. I authorize the athlete to attend tryouts and, if selected, participate in the registered sport with Cheyenne Youth Sports League, Inc. I will promptly update the league about changes to contact details or participation restrictions.
2. Tryouts and team selection
Registration and attendance at tryouts do not guarantee a place on a team. Not all youth athletes who try out will be selected. Coaches and members of the Board will decide which players are selected, considering evaluations, age eligibility, available roster spaces, and team needs, and following the league’s conflict-of-interest policy. Signing up does not guarantee playing time or a particular position.
3. Acknowledgment of injury risks
Sports involve inherent risks that cannot be completely eliminated. Volleyball risks include collisions, falls, ball impacts, and injuries to fingers, wrists, ankles, knees, shoulders, and the head. Baseball risks include being struck by a bat or ball, collisions, sliding, falls, throwing injuries, and weather exposure. In either sport, serious injury, concussion, permanent disability, or death is possible. I understand these risks and voluntarily accept the inherent risks of participation to the extent permitted by Wyoming law.
4. Limited release of my own claims
In consideration of the athlete being permitted to participate, to the extent permitted by law, I release Cheyenne Youth Sports League, Inc., its directors, officers, coaches, employees, and volunteers from my own claims arising out of participation, including claims caused by their ordinary negligence. This release does not cover gross negligence, recklessness, willful or wanton misconduct, intentional acts, or any liability that cannot lawfully be released. It does not purport to waive the athlete’s independent legal rights where a parent or guardian cannot lawfully do so.
5. Health, injury reporting, and emergency care
I will provide the league’s full medical and emergency-contact form before the athlete participates and notify coaches of relevant conditions or restrictions privately. If I cannot be reached promptly, I authorize league personnel to obtain emergency medical assistance and transport for the athlete and authorize necessary emergency care by licensed providers. I am responsible for medical costs to the extent required by law. An athlete with a suspected concussion must be removed immediately, may not return the same day, and must have written clearance from a qualified healthcare provider before returning. I will report relevant injuries, including those occurring outside league activities.
6. Permission for travel within Wyoming
I understand that the team will travel across Wyoming for games, tournaments, and other league activities. I give permission for the athlete to attend those activities, subject to the itinerary and arrangements shared for each event. Transportation is ordinarily the family’s responsibility. This permission does not authorize the athlete to ride with an unspecified driver or approve an overnight stay. Any league-arranged ride, designated driver, overnight stay, or travel outside Wyoming requires separate arrangements and my specific consent. I will tell the league promptly if the athlete cannot attend an event.
7. Safety and conduct
The athlete and I agree to follow league rules, facility rules, coaches’ safety instructions, and the applicable Codes of Conduct. Bullying, harassment, discrimination, threats, and unsafe behavior are not acceptable. The athlete must use required protective equipment and report injuries. Conduct concerns may result in removal from an activity or further discipline under league policy. We will complete the required code-of-conduct acknowledgments before participation.
8. Fees and participation paperwork
This online submission is a tryout or registration request, not confirmation of team placement or a payment. The league will communicate any fees, deadlines, financial assistance, and refund terms before payment is due. A full registration and waiver, medical and emergency-contact information, and any additional event-specific permissions must be completed before participation. No payment information is collected here.
9. Contact information and photographs
I authorize the league to use the information submitted here to manage registration, contact me about tryouts and participation, and document my permissions. This does not provide consent for advertising messages or publication of the athlete’s photograph. Photo and video preferences are collected separately on the full registration form. I may contact the league to request a correction or deletion of registration information, subject to records it must retain.
10. Electronic acknowledgment
By entering my name and selecting the acknowledgments, I intend my submission to serve as my electronic signature to these terms. I have read the terms, may save or print a copy, and may contact the league with questions before submitting. A record of the terms version, permissions, and submission time will be retained. If one provision is unenforceable, the remaining provisions continue to apply to the extent permitted by law.
